New Teacher Grant Application

We offer one-time grants for every new teacher. Please fill out the form below and if approved, your check will be mailed to you.

  • Registration and School Information

    (Electronic transfer requires that you give your account information to the EPAK Treasurer by email or phone. Instructions will be emailed to you once your grant is approved.)
  • Disclosures

    To maintain our tax-exempt status EPAK is required to keep records of your relationship to EPAK board members, officers or donors.
  • Certification & Agreement

    I certify that I am directly responsible for the application and use of these funds. If I am awarded this grant, I agree to:
    1. I will submit a post-project report along with scans of original receipts and tallied to allocations@epak.org by December 15 (Fall) or June 15 (Spring).
    2. Return unused funds to EPAK or request in writing before using them.
    3. Leave any books, equipment, and materials purchased with these funds with my school or institution if I leave so that the children will continue to have the benefit of this grant.
    4. Not apply again until all receipts from prior projects have been sent in.
    5. I have read the grant parameters on the EPAK website and agree to abide by them when I apply for fall and spring grants.
